Hospital Quality Incentive Payment (HQIP) Governance Charter Timeline   This board approved the measures that will be collected back in August 2015 Model Year 2016-17 Base Measures o Emergency Department Process effective dates January 2016 through December 2016 o Cesarean Section data on calendar year 2015 o 30-Day All-Cause Readmissions for calendar year 2015 o HCAHPS for the four months prior to July and then the twelve months after that o Culture of Safety with four options within that, dated January 2015 through April 2016  Model Year 2016-17 Optional Measures o Active Participation in Regional Care Collaborative Organizations (RCCOs) for service dates for calendar year 2016 o Advance Care Planning for service dates for calendar year 2015 o Tobacco Screening for service dates for calendar year 2015  Model Year 2016-17 Maintenance Measures o Post-operative pulmonary edema/deep vein thrombosis (PPE/DVT) for service dates for calendar year 2015 o Central line-associated blood stream infection (CLABSI) service dates August 2015 through July 2016 o Early Elective Deliveries for service dates for calendar year 2015   Data collection will go on through the month of May We will then consolidate and quality check the data before sending it to the providers to for review during the validation process in June Goal to have the data finalized by August 7th We will present the scoring and bucket options to this board on August 23rd Goal is to have scoring finalized by September 15th Would like to have the 2017 measure finalized by this board by September 2016 as well as the 2018 measures Not looking at making significant changes to the measures in 2017 The purpose of the HQIP Committee is to recommend to this board the components and measures of the HQIP program including how the measures will be scored, the methodology, and the data sources Under this new governance the HQIP Committee will need to form and coordinate an advisory group        Our mission is to
