KCC Club T-Shirt Order Form
In order to purchase t-shirts for your club, complete this form and email to studentli[email protected].
Once this form is submitted, no changes may be made.
The Office of Student life will notify the primary club contact and advisor when the t-shirt design
and order are ready for approval. Once the club and advisor approve the design and cost, the
order will be submitted. Expect delivery between 8-10 weeks after final approval and the order is
placed.

The more imprint locations you choose, the more expensive it will be. If your club is creating the
t-shirt design, email it, in Word or JPEG format, as an attachment with this form and specify
imprint locations (front, back, sleeve, etc.) and imprint size. Maximum imprint size is 12”x12” for
designs on front and back of shirt.
Our club is creating the t-shirt design and will email with this form.
Our club prefers a KCC graphic artist create the t-shirt design.

Specify what color you would like your t-shirts to be. If ordering less than 40 t-shirts, choose one
shirt color. For orders of 40 or more, you can order up to 3 shirt colors. See color chart below.
