The consular outreach program is being undertaken in cooperation with the Philippine Honorary Consul in Texas and the
President of the Philippine American Chamber of Commerce of Texas (PACC Texas) Ms. Ethel Mercado, and Ms.
Myrna Carreon, PACC Region Chair.
All Consular Service applicants are reminded of the following:
STRICTLY BY APPOINTMENT. NO WALK-IN APPLICANTS WILL BE ACCEPTED.
APPLICATIONS THAT ARE INCOMPLETE, INCORRECT OR SUBMITTED THROUGH THIRD PARTY
FACILITATORS (ex. NOTARY PUBLIC) WILL NOT BE ACCEPTED.
APPLICANTS MUST PERSONALLY APPEAR 15 MINUTES BEFORE THE APPOINTED TIME AND BRING ALL
THEIR ORIGINAL DOCUMENTS.
FAILURE TO APPEAR DURING THE APPOINTED TIME OR TO BRING COMPLETE DOCUMENTS MAY RESULT
IN THE APPLICANT’S EXCLUSION IN ALL CONSULAR OUTREACH SERVICES IN TEXAS FOR 2016.
THE CONSULATE DOES NOT CHARGE FEES NOR ACCEPTS DONATION RELATED TO THE ENTRY OR USE
OF THE VENUE

Note:
1. Personal appearance is required in all cases (including applicants who are 65 years old and above and minors
who are below 18 years old).
2. Do not bring passport pictures.
3. The applicant must wear decent attire (no sleeveless and/or collarless attire) and without eyeglasses/contact
lenses.
4. No facial piercings allowed.
