Student Volunteer Scholarship
INSTRUCTIONS:
Prospective applicants must be a senior at one of the high schools located in our service
areas, and be an active Halifax Health - Hospice Volunteer. Additional criteria to be
considered include community service, demonstrated leadership in school, character,
integrity, initiative and other personal qualities. In addition, the following documentation
must be attached to this application form:
REQUIREMENTS:
• Must have a minimum of 75 volunteer hours with Halifax Health - Hospice
• Minimum 3.0 GPA.
• Essay submission deadline is April 1, 2016.
• Essays can be no more that 750 words but must be a minimum of 500 words.
• Essays must be original work of the student.
• Essay topic: What it means to you to be a Hospice Volunteer, and how it will
impact what you want to do in the future?
• One letter of recommendation from a teacher or counselor at the student’s
current school on letterhead.
• One letter of recommendation from a person or organization that is closely
familiar with the applicant’s community service, leadership activities or
charity work, on letterhead.
• Verification of College acceptance.
