Human Subjects Training Certification Letter
To comply with the NIH policy requiring education on the protection of human subjects, University of
Wisconsin-Madison certifies that the key personnel listed below, who are involved with human subjects
or the design and conduct of the proposed study, have completed an educational program entitled
“Human Subjects Protection Tutorial at the University of Wisconsin-Madison.”
The tutorial is adapted from the NIH Computer Based Training on Protection of Human Subjects and is
available at https://my.gradsch.wisc.edu/citi/index.php. The tutorial includes training on the
responsibilities of research investigators and Institutional Review Boards in proper conduct of research
involving human subjects based on ethical principles described in the Belmont Report, Federal
regulations and the University of Wisconsin-Madison’s Multiple Project Assurance.
