The November Medication Events trend was
reviewed.
MERP element trending report was reviewed.
Issues of note included:
There was a 52% increase in the number of
incident reports.
No reports resulted in patient harm.
There was a 391% increase in the number of
prescription order communication/
documenting errors. A large number of
these reports were related to uncharted
controlled substances.
ADR and Insulin related event trending was
reviewed. Issues of note included:
There were 13 Adverse Drug Reaction
incident reports in November.
There were 41 Hypoglycemic Related Event
incident reports in November.

be trended monthly.
MERP elements will continue to
be monitored.
Uncharted controlled substances
issue was presented to Nursing
Management Council. It was
recommended that nurses print
activity reports at the end of their
shifts to confirm all charting of
controlled substances is done.
Continue to monitor and review at
next MSC meeting.
This report will continue to be
updated monthly.
