(INSERT) Patient's signature if you are complaining on behalf of someone else OR include a
consent form OR if the patient is unable to give consent e.g. too young, ill or deceased you should
explain this in the main body of the letter.
(INSERT) Details of any documents you are sending with your letter e.g. Diary of events.
(INSERT) Request for copies of the response letter to be sent to other people Please also send
copies of your response to this letter and copies of all other future correspondence to (e.g. Mr
Smith, ICAS Advocate (insert postal address).
