Class T-Shirt Order Form
Havens Elementary will again be purchasing class T-shirts for our students and teachers. Each
class will have its own tie-dyed color. We will be wearing these shirts for field trips, Fun Day,
Track and Field Day, and other events throughout the school year. These T-shirts can also be
used as your child’s gym shirt (K-2
nd
grade). There will be a special spot to write each child’s first name on
the front of the T-shirt.
The cost of this T-shirt is $10. Please complete the form below
and
return to school by September 11
th
.
