Dear Interested Applicant,
Thank you for your interest in our Mutual Self-Help Housing Program. In addition to your loan application, there
are a few other forms that will need to be filled out in order to completely process your application. If you have any
questions while filling out the application please contact me by calling 435-753-1112.
Additional information to be included with the application:
Signed copies of the “Authorization to Release Information” for all individuals in the household 18 years or older
Signed copy of “Employment and Asset Certification”
Copy of the last two years federal income tax returns for all working individuals 18 yrs or older living in the
home
Copy of the last two years of W-2 forms
Copy of the last four pay stubs for each income earner for each job if working at more than one place of
employment
-for Self-Employed individuals other documentation may be required
Copy of 2 months bank statements for each open bank account you have
Copy of a photo i.d. (driver’s license) and your social security card
€ If you receive or pay child support, receive social security benefits or income from an unusual source, have filed
bankruptcy, pay for child care, have been divorced, are selling or have recently sold your home, have recently
paid off one or more of your debts, are not a US Citizen, or have student loans in deferment, you’ll need to bring
in additional documentation.
If you’re a full-time student you’ll need to write and sign a letter stating that you intend to stay in the area after
graduating.
A copy of your household budget including all income and expenses.

• Check, Money Order, or Credit Card Acceptable
• Please make checks payable to Neighborhood Nonprofit Housing Corporation
Check for credit report fee:
• $32.00 Single or Married
• Please make checks payable to USDA/Rural Development.

Please note that your space in the program will be based on a first come first serve basis. In order to secure your
spot on the waiting list you will need to get all the necessary information to NNHC as soon as it is asked for or else
someone else may be able to slip in and take the remaining available spots. Final determination of program
eligibility will be made by the USDA Rural Development. This application only guarantees your spot if you receive
final eligibility from the USDA Rural Development. Thank you for your cooperation in procession your loan
